Jurors scheduled for service beginning in January 2015, your reporting instructions will not be available until December 29, 2014.

Postponement or Excuse Request

You may request to be exempted or excused from service or to have your service postponed. Click the above link if you wish to do so. You must appear for jury service, unless you are notified that the Court has granted your request. We must receive your completed questionnaire prior to submitting your excuse or postponement request. You can check the status of your request by accessing your reporting instructions via phone or web.

Helpful Information:

Notice to Jurors:

The Federal Courts do not require anyone to provide any sensitive information in a telephone call. Most of our correspondence with you will be through the U.S. Mail. If there is any phone contact, none of our staff will be requesting social security numbers, credit card numbers, or any other sensitive information.

Persons receiving such a telephone call should not provide the requested information, and should notify the U.S. District Clerk's Office in their area.

If you have received a juror qualification questionnaire from our office, a listing of commonly asked questions have been compiled. Please reference this .pdf to find the answer to one of your questions.

The below Juror Qualification Questionnaire is sent out to determine if a person is qualified to serve as a juror.

Below are some important points to note when you receive this questionnaire from our office.

On questions 3, 8 and 14, you may need to include further written clarification in the "Remarks" section of the form (located on back of form).

On question 14, you will find three categories of persons who may be excused from service as a juror. If you fall into one of these categories, you should fill in only bubbles 1, 2 or 3. The additional bubbles do not apply to our district.

Grounds for Requesting Excuse

If you have recently received a Juror Qualification Questionnaire Scantron Form and you have questions about filling it out, you can either e-mail the Jury Help Desk reqarding your Juror Qualification Questionnaire, or you can call our toll-free telephone number at (866) 279-7008. The phone line is available Monday through Friday, from 8:00 a.m. to 5:00 p.m. (Central Standard Time).
