Juror Qualification Questionnaire
The below Juror Qualification Questionnaire is sent out to determine if a person is qualified to serve as a juror.
Below are some important points to note when you receive this questionnaire from our office.
On questions 3, 8 and 14, you may need to include further written clarification in the "Remarks" section of the form (located on back of form).
On question 14, you will find three categories of persons who may be excused from service as a juror. If you fall into one of these categories, you should fill in only bubbles 1, 2 or 3. The additional bubbles do not apply to our district.
