Notice to Jurors:

The Federal Courts do not require anyone to provide any sensitive information in a telephone call. Most of our correspondence with you will be through the U.S. Mail. If there is any phone contact, none of our staff will be requesting social security numbers, credit card numbers, or any other sensitive information.

Persons receiving such a telephone call should not provide the requested information, and should notify the U.S. District Clerk's Office in their area.

Juror Qualification Questionnaire

If you have received a juror qualification questionnaire from our office, a listing of commonly asked questions have been compiled. Please reference this .pdf to find the answer to one of your questions.

The Juror Qualification Questionnaire is sent out to determine if a person is qualified to serve as a juror.

Below are some important points to note when you receive this questionnaire from our office:

  1. If you are summoned for jury service, you will report to the divisional office listed at the top of the questionnaire form.
  2. If the listed address is not your current address, cross out the address and print out your current permanent address.
  3. On questions 3, 8 and 14, you may need to include further written clarification in the "Remarks" section of the form (located on back of form).
  4. On question 14, you will find three categories of persons who may be excused from service as a juror. If you fall into one of these categories, you should fill in only bubbles 1, 2 or 3. The bubbles are (1) Persons over seventy (70) years of age (if so, give month, day and year of birth under "Remarks"); (2) Any student enrolled in a public or private secondary school and any full-time student enrolled at an institution of higher education; and (3) Persons who serve without compensation as a volunteer firefighter or member of a rescue squad or ambulance crew for a federal, state or local government agency (Describe your service and identify the agency for which you work under "Remarks").

    The additional bubbles do not apply to our district.
